The challenge
Patients with inflammatory arthritis managed on Disease-Modifying Anti-Rheumatic Drugs (DMARDs) require regular blood test monitoring to ensure safe, continued use. Traditionally managed in secondary care, this created bottlenecks: as many as two thirds of rheumatology outpatient appointments were for monitoring only, creating long waiting lists and inefficient use of consultant time. Patients who were stable were attending hospital unnecessarily, while new patients waited longer for access.
Our solution
Medacy piloted a community-based DMARD monitoring service in North Buckinghamshire, with specialist clinical pharmacists based in primary care taking over monitoring responsibilities for stable patients. Working alongside secondary care consultants and aligned with the NHS Long Term Plan’s focus on out-of-hospital care, pharmacists ensured timely blood tests, acted on abnormalities, followed up missed tests and provided direct patient counselling on therapy adherence. During the COVID-19 pandemic, the model proved particularly resilient — pharmacists built a centralised data system to identify and prioritise all high-risk patients during the outbreak.
